Prequel about teenage Leatherface who escapes from a mental hospital with three other inmates, kidnaps a young nurse and takes her on a road trip from hell. 1955 年德州設立受害兒童援助計劃。第一年有接近50 名兒童從家中被帶走到特設的院舍，這些家庭大都是因為有犯罪行為、基因缺憾或者是有不當行為。 一名年輕女護士被四名從院舍逃走的暴力青年綁架，最終令她走上不歸路。路上他們被同樣是精神有問題，報仇心切的警察追捕。其中一名恐慄的青年人注定以悲劇收場，他的意志被徹底摧毀，最終使他蛻變成現時我們口中的殺人狂魔「皮臉」。
